Your feedback
Your views on how we can improve our service are important to us. You can give feedback directly to the Hammersmith or Fulham centres for health by talking to a member of staff, completing a feedback form or by contacting reception on 020 8893 0384.
There are also several ways you can get involved and have your say on local health issues. You can:
• Respond to our consultations and surveys
• Make a complaint if you are unhappy about the care you received

Partnership for Health will consult with local people on any significant changes to local health services.
